How to get the Application Form and Guidelines for the Master's and Doctoral Programs of the Graduate School of Medicine

(1) Application Form/Guidelines

Request the form by post in an envelope clearly stating “Request for Application Form and Guidelines for the Master's (or Doctoral) Program of the Graduate School of Medicine” in red. Enclose a self-addressed envelope with 320yen in stamps on an (Kakugata No2) envelope. Be careful to specify whether the request is for the Master's or doctoral program materials, also write in your contact phone number, and e-mail address. Send the request to the contact address below.

(2) Request for past entrance examinations questions (last three years)

Request past exams by post in an envelope clearly stating “Request for past exams for the Master's (or Doctoral) Program of the Graduate School of Medicine” in red. Enclose a self-addressed envelope with 320yen in stamps on an (Kakugata No2) envelope. Be careful to specify whether the request is for the Master's or doctoral program materials, also write in your contact phone number, and e-mail address. Send the request to the contact address below.

(3) Request for both of the Application Form/Guidelines AND the past entrance examinations (last three years)

Request these by post in an envelope clearly stating “Request for Application Form/Guidelines AND past exams for the Master's (or Doctoral) Program of the Graduate School of Medicine” in red. Enclose a self-addressed envelope with 510yen in stamps on an (Kakugata No2) envelope. Be careful to specify whether the request is for the Master's or doctoral program materials, also write in your contact phone number, and e-mail address. Send the request to the contact address below.

During office hours (weekdays from 8:30 a.m. to 5:00 p.m.), Requests for information materials can be made at the counter.
